If you are a medical student, please answer these:

4. Your undergraduate major (and minors, if any)?
5. Desired specialty?
6. MCAT Score (If you’re willing)?
7. Undergraduate GPA and class rank (if available)?
If you are a medical resident, please answer all the above (1 through 7), plus these:
8. Where are you doing your residency?
9. How did you get into this residency program?
